New Client - please submit a separate form for each person who will be attending the sessions
Group Request - if you are interested in one of our groups, please complete this form to request more information and we will contact you to complete registration
Guarantor - please complete this form if you are the financially responsibility party for sessions
Consent To Treat A Minor - for authorization from parents, legal guardians, and conservators permitting psychotherapy services for a minor
Authorization for Release of Information - consent to speak with others such as medical or legal professionals, family members, schools, or whomever you choose to support your therapy process
